General Information
You must complete this form and provide all documents that apply to your situation in the format required, otherwise your application will be rejected.
The form to submit documents in support of the application for Quebec Selection Certificate defines the nature requirements, the format and the translation of the documents you must send us.
This form must accompany your certificate request selection.
Only you, as the principal applicant or principal applicant, must complete, even if your spouse or a dependent child with you.
Instructions
1. Check the box next to each document you submit. The NA mention (not applicable) indicates that you do not have to submit this document.
2. Gather all the documents requested in the format required (original, certified copy or photocopy).
3. Place all your documents in the order indicated in the form.

Important
Certified copy
The format "certified copy" indicates that the copy of the document you submit must be certified true to the original either by the issuer of the document or by a recognized authority of the country or territory that issued the document.

Translation
If you submit documents or parts of documents in a language other than French or English, you must provide:
• the title in the original language, in the format required (original, certified copy or photocopy);
• the original of the complete translation by a qualified translator. For requirements related to translations and definition a recognized translator, visit our website at www.immigration-quebec.gouv.qc.ca/exigences-documentaires.
If the original document is written in English or French but it includes a seal in another language, you must absolutely translate this seal.
Audit and Investigation
You should know that the Department can:
• check or have a third party confirm the accuracy of the information you provide and if you contact the Ministry any information that you know or should have known to be false or misleading with respect to your selection certificate application, you
commit an offense against the law and you are liable to a fine;
• reject any application that contains information or a false or misleading document;
• refuse to consider your selection certificate application if you have provided for five years or less, information or false or misleading document on an application you have made under the Immigration Act in Québec;
• cancel a selection certificate if it was issued on the basis of an application that contains information or a false document or misleading, if it was issued in error or if the conditions for its issue no longer exist.

If your profession or trade is governed by a professional order or any agency regulations in Quebec, you must provide the statement applies to your situation, after having dated and signed.

If applicable, your spouse or jointly must also provide this declaration.

Identity and civil status

10. • At least two items attesting to your status as de facto spouse on which your name and the name of your spouse appear. The following documents are eligible: Photocopy
– Mortgage or dwelling lease
– Statements of joint bank accounts
– Utility bills (electricity, natural gas, telephone or Internet service)
If you and your spouse apply as de facto spouses, you must prove that you have co-habited continuously for at least 12 months.
You must provide a document that goes back more than one year and a document with a recent date
Resident status or temporary resident in a country other than that in which you are a permanent resident or a citizen or permanent resident or citizen

11. • Residence permit or stay document with a cumulative validity covering at least once year, issued by authorities of the country or territory where you currently reside. Photocopy

Dependent children accompanying you

15. • Parental Consent Statement made before a notary, magistrate, judge, a judicial officer, an employee of a government department or agency or a diplomatic or consular representative of your country or territory citizenship which must contain the following items: Original
- The name and surname of the child;
- The date of birth of the child;
- The name and the first name of accompanying parent (you or your spouse or joint);
- The agreement of the non-accompanying parent expressly authorizing immigration her child permanently in Québec;
- The name, first name and contact details of the non-accompanying parent, in letters printing;
- The signing of the non-accompanying parent;
- The date of the declaration;

Education and training

Eligible diplomas are:

21. Each job reported in the selection certificate application:

Each certificate or working certificate must be submitted on the letterhead of the company and must contain the following information:
- The duration of employment (start date and end date of employment);
- The number of hours worked per week;
- The positions you have held and the period during which you held each position;
- A brief description of duties;
- The employer's contact information (including phone number);
- The name of the person signing written in capital letters;
- The signature of the person signing and the date;
Proof of legality of your experience may be you requested at any time during the processing of your request.
Employment contracts and hiring promises are not accepted.
Work internships as part of studies over the last five years

22. For each course stated in the selection certificate application: Photocopy

Each course certificate must be submitted on paper to the company's letterhead and must contain the following information:
- The training period (start date and end date);
- The main tasks and responsibilities;

- The company's contact information (including phone number);
- The name of the person signing written in capital letters;
- The signature of the person signing and the date.

25. • Proof of legality of work experience Notice of assessment to tax on personal income. This is an official document issued by the tax authorities confirming the receipt of the tax return and evaluation of the information submitted. Original or Certified True Copy
Important clarification on the proof of legality:
If you work or have worked in a country or territory other than one in which you are a permanent resident or citizen or permanent resident or citizen, you must submit a photocopy of your work permit.
Authorization to practice a profession or trade regulated and Mutual Recognition Arrangement (MRA) between France and Quebec

26. • Exercise authorization issued by a Québec regulatory body. Photocopy

• Certificate of qualification or competency issued by the body regulatory Quebec
You must provide one of these Documents if you exercise a regulated trade for which an arrangement mutual recognition (MRA) was signed between France and Quebec.
Language testing in French
