Is US PCC required even if SSN is not available ?

I am an Indian and I currently live in India. I have applied for Canadian PR and I recently got the ITA.
I have been to US through B1 visa twice. First time i stayed there for 90 days and the second time (after 1 year) I stayed there for 91 days. I do not have Social Security Number.
In the website link to get the US PCC

in its form, it asks for the last 4 digit of the SSN. Does it mean that only people who have SSN number should apply for US PCC ?
Please clarify.

You need to get US PCC . Leave the SSN field blank or write NA.

Does she also require US PCC ?

Obvious.. if she hadn't visited, PCC is not needed.

In whose name should the "Certified Check" be taken to apply for US PCC ?

The "Identity History Summary Checks Review" form says it is USD $18 as fees and the address as "FBI CJIS Division – Summary Request 1000 Custer Hollow Road Clarksburg, West Virginia 26306".
