Hi guys. I got my medical done on November 16th 2016. I submitted my Application for permanent residency through CEC (Express Entry) about a month after that. Unfortunately my application was "canceled" several months later due me not realizing that my IELTS certificate had expired by a few months.

I had to make a new express entry profile and while checking all my documents I noticed that my medical print out makes reference to my old Express Entry number. This doesn't match my new Express entry number for this application. My medical is still within the 12 months and it has all my details on it but I don't want to be rejected again so I am trying to find out if I need a new medical or not. Any ideas? Thank you in advance.

As long as you get ITA this month & submit your eAPR, you should be fine. CIC usually requires medical to be valid for 6 or more months while processing EE PR application.

They used to, but not anymore.

See http://www.cic.gc.ca/english/information/medical/medexams-perm.asp under Validity.
