Hi All, your expert advises are badly needed. We're confused about the following questions in spousal application form.

IMM5490E 1. Did you have any contact with your sponsor before you met in person?
Question: My husband and I are confused with this question 1 in IMM5490E. Is it our first contact when I am in Canada or the date when we became boyfriend/girlfriend or the date when I became his spouse?

IMM5490E 2. When and where did you first meet your sponsor?
Question: Is it the first time we met as boyfriend and girlfriend or the day I became his spouse?

Additional question:

I am recently married and I am going to sponsor my husband, should I indicate my family name at birth exactly as it appears on my passport or should I use my spouse’s surname?

The questions are pretty straightforward.
1. It's asking if you talked to your sponsor before meeting in person. In my case, I said yes, because we had exchanged e-mails.
2. If you met your sponsor on the day you got married, then write that. If that's the case, then CIC will raise an eyebrow, because most people know their spouse before marrying them. It's asking about the first time you met them, not as a spouse or a boyfriend, but meeting them as a person for the first time.

The guide tells you to fill out of the forms with the information as it appears in your passport.
 
Thanks for your answers Milesaway. Actually, after 14years of our relationship we just got married last month. I went home in my country for our wedding and I am still here helping my husband to fill out the forms.. I'll bring all the forms when i return to Canada.

I have one more question, how about the medical exam for my husband, am I going to submit it together with the application or should my husband wait for CIC's notice for medical exam after we submit all the application forms?

Sorry for asking too many questions because I am not familiar with the process in spousal sponsorship.
