Please help - I am confused about how to enter information about a former common law partner in my PR application.

I have been living in canada for 4 years (work visa) and had a common law partner whilst in canada for the first 2 years (a Canadian citizen).
We are no longer together (as of 2 years ago).

On the General Application form I have included her in section 14 (previous common law partner).

However, I am confused about form IM 5406 "Additional Family Information".

1) Should I enter her in Section A on line 2 ? There is no-where to indicate that she is a former common law partner!
OR
Should I sign the Note 1 to say that I dont have a spouse or common law partner (although it also says DONT sign if you have a former partner).

OR, should I just take her off the application completely since she now has nothing to do with me or my application?
