In Ontario, where should "out of province form" medical cost mail to?

I am living in Ontario. I went to an emergence department when I was travelling outside of Canada.

Upon returning to Ontario Canada, to get refund, I filled the form - Out of Province - Country Claim Submission

But I am not sure where should I mail the filled form to.

Could anybody please give me some hints? Thank you guys very much. Have a nice long weekend.

There are 3 addresses for Claims Processing offices at the bottom of the form, maybe mail it to one that is closest to you? Otherwise, try calling Service Ontario and see if they can direct you.
