Shaw, the information is on the page where the checklist is if you have to date, sign the ID photos etc, and not on the Appendix B which is the same for all applicants (I think) . Which country are you applying from?

yes you are right. I am not from Philipines. I see now Philipines applicants need to sing, seal and date.

These forms are confusing considering they are different for every country LOL Out of curiosity I checked the Country Specific Guide for Pakistan, and they must provide a lot of extra forms and information, therefore their process is also so long for processing as that VO checks every little detail.
