IMM5710 form questions

Hi! so my husband and I are going through the sponsorship application together right now and making sure we have every thing ready to send away.
Now we just noticed that our IMM5710 form was not validated. We're not sure what to put down for the dates for duration of employment since we have no idea when he will be accepted for his open work permit? We don't have a particular employer or any thing so we put down unknown.

Also, we read on here that we will also have to pay the open work permit fees. We see there are two similar looking fees (Work permit including extensions – per person $155) and (Open work permit holder - $100)
we arent sure which one applies to him.

sorry I should have put down the form we are filling out is IMM5710 for OWP while he's a temporary visitor until October 1st.
we want to send it out with our sponsorship application. We paid for the PR fees as well (the $1,040 option) but are not sure if we need to fill out a PR application, too?
