On that sections it says:
Starting with your current employer, if applicable, give details of all employers you have worked for over the past five years. Make sure there are no gaps
Within the last 5 years:
I've had two full time jobs (easy).
I've had an internship.
Two simultaneous part time jobs, one of them while in the internship.

The part-time jobs and internships were paid hourly. I don't know the exact "Monthly gross salary/income". Should I write an approximate amount?

For the part-time job I also don't remember the exact start date and don't have the contact information. What to do?

Finally, the form only provides 5 Fields. There is no enough space to add an unemployment gap I had for about 4 months. Should I skip it?

Yes write an approximate amount based on your average hours per month.

They are asking dates as YYYY-MM, you cannot remember the month you started and finished your jobs? Maybe check on your T4 forms..?

NO! They wrote on bold "Make sure there are no gaps". Also, at the top, they wrote "If you need more space for any of the questions, use another sheet and write the part and question number". Then that's what you have to do.

I am facing the same problem, i also did two hourly part time jobs simultaneous - one was a day job and one was a night job.
My question is..
How to manage time line in Date field for more than one job for same period?
How to write dates if i had job only for 1 month?
Eg. 2017-09 to 2017-10 - work for subway
2017-06 to 2017-09 work for starline food
2017-08 to 2017-10 work for wendy(night shift)
Note - In 09th month, For half a month I worked at subway and for half a month at starline food.
Please help.
