IMM5406E additional family info confusion of filling applicant and spouse name

I tried to listen to this video on the link many times but something wrong with it
can you kindly summarize the instruction given on this tutorial mainly for our point of discussion

The text is as follows:
"...So if you are applying with a spouse or a common-law partner you'd have two of these forms: in the first form the principal applicant would put their name first beside "Applicant" but in the second form the spouse or common-law partner would put their name in front of "Applicant" and the principal applicant would be their spouse or common-law partner. This makes it easy for CIC to recognize that the form is of the spouse or common-law partner. So, for example, if John Smith is the applicant, on one form he would be John Smith - Applicant, and then on his spouse's or common-law partner's form John Smith would be in the Spouse or the Common-law partner line, and his spouse, let's say it's Jane Smith, would be on the Applicant line..."

Hi Guys... we lost our chance as we got rejected saying that the Spouse's additional family information wasnt provided.
However it was there and i feel the video is right because it seems very confusing to the cic so better change the names in the field of applicant receptively.
Its such a shame that our form was filled by an agent...

But this is exactly what I have said. Screw agents. Read instructions carefully, never listen to rumors or follow one's advice without having made a good double or triple check.

Now go to EOI/EE system. I hope it will work fine and reduce the amount of such mistakes and misunderstanding.

Hi everyone.

I hope Roman can help me as it is related to Russian names including Patronymic name. I have an issue with it in this application form. I would like to know, whether I must to write my patronymic name or not. To add, I didn't mention it in IMM0008 application form, just given name and family name.
