I'm the principal applicant (Canadian Citizen). Concerning this document, is it my spouse that fills it out?

If I well understood, in the applicant field, he will put his name, spouse or common-law, he puts his name once more and in the parents section, it's his parents information.

I would say that you are the sponsor and your spouse is the principal applicant.

For IMM 5406, your spouse is the applicant, you would be his spouse/common-law partner, and then the parents/children/sibling information is that of your spouse.
