IMM5406 returned in PGP application

I applied for parents' PR under PGP in July 2024 - father is principal applicant and mother is secondary applicant.

IMM5406 got sent back on 22 Nov 2024 stating it's incomplete. It was an online form and not a pdf and there were separate sections for principal applicant's (my father's) family and dependent's (my mother's) family. So, all my father's family info (parents, children, and siblings) and all my mother's family info (parents, children, and siblings) were included in that online form.

So, I am not sure what was missing. The only thing I could think of is that I filled my grandmother's name in the required field with just the first name (not first name + last name) just as it is on my mother's official documents. The letter received from IRCC doesn't mention any specific reason though.

Anyone has any clue what could be wrong?

They should each fill out one form - separate 5406s for adults.

I should clarify that the online form (not pdf but online fillable) is indeed separated for the 2 applicants. So, effectively, I did fill out separate forms for them.
Ah, didn't know that. Mea culpa. Sorry, I don't know what the issue is.

Is there a way to get pinpoint information on what they think is missing? I could raise a webform but I have heard they generally respond with generic information.

It's hard to tell what you've missed.
One common mistake is not including yourself in the list of children. If you have already include yourself in both forms (IMM5406 for your father and mother), then I don't know what else could be wrong.
You can try to call IRCC call centre but I am not sure if they can point out exactly what is wrong.
