IMM008 - Generic Application Form of Canada

I am pretty confused as to who should be doing this form, on the document checklist it says the sponsored person should be filling out this form and on the cic website it says you the applicant? that could me mean either me or her but there is someone confusing wordings in the form, so who fills out the generic application form?

like at the top it says Personal details and question 13 (c) says provide the name of your current spouse/common law partner, so does either one need be different if she is filling the app it would be her personal details and full name and 13 c would mean she would put my name am I correct?

Sponsored person is the main applicant. So this form is for her


When form asks about HER spouse, she gives your name.
