IMM 5481 q's: middle names and education?

Wow awesome forum, appreciate the help for something as puzzling as the immigration docs.

My questions are fairly pedestrian, but they seem to be a stickler for details:

-Whenever the docs ask for my "given name", do I need to include my middle names(s)? My passport doesn't have a middle name on it, not sure if Immigration Canada wants it?

-For highest education, I graduated high school and did one of those new media under 1 year courses at a specialized school. No "post grad" or "college/university." Does it make sense to just check off high school? Or am I supposed to check off everything that I've completed at all?

-Can I just walk into a CRA office to request an Option C? Or is the phone the only way to get it?

1. Put whatever is on your passport to confirm your personal information.
2. Choose your highest, completed level of education (high school for you).
3. I don't see why you can't walk-in and get an Option-C but calling them seems to be the 'standard' way to obtain one.

Oh, one other question...

-Where it says to indicate employers over the past 5 years, it asks for monthly income. Can I simply write the monthly gross income? (eg: yearly/12) I have one of those jobs that pays every few weeks, not twice a month exactly, and the document doesn't seem to specify what kind of income it's looking for (gross or net.)

I wrote down my gross salary for 4 weeks. Don't know if it's right but I guess I'll find out if it's wrong!
