I'm a little stumped on the income questions. On the Option C printout my NET INCOME is much higher than my actual NET income is because I pay into a pension, I pay Union dues, etc.

So on question 3 - shall I put my ACTUAL net income, as in money in my pocket after 12 months or should I include my pension contributions in my income?

I will also be providing an Option C printout for 2012 so they will likely understand but I don't want my application sent back on a technicality...

Does your pay statement have a line item called 'Net Income'? Does it also increment a 'year-to-date' value? I used those numbers. Mind you I did calculate a true "preceding 12 months" by doing some adding and subtracting from the previous years' FINAL year-to-date 'net income'.

I used my year to date gross and then minused my year to date deductions. They will have both documents to work from so they will figure it out.
