Hi all

This is a question regarding IMM 5406 (Additional Family Information).

The guide gives the following directions:

Additional family information (IMM 5406)
Who needs to fill out this application form?

This form must be completed by:

You, as the principal applicant,
Your spouse or common-law partner (whether accompanying you to Canada or not), and
Your dependent children aged 18 or over (whether accompanying you to Canada or not).
My question is, does this mean that each of those listed above must complete their own version of the form? Or those listed above must fill out their respective sections of one form? ie, my wife is a Canadian citizen (she is sponsoring me so I am the "Applicant").

Currently I have ONE copy of IMM 5406 with information filled in for myself as Applicant, my wife (under Spouse), my mother, my father, my wife's two children (I am not their birth father so I wrote Step-son/Step-daughter), and my sister. Is this correct and good to go, or do I need to create a second copy of this form to include my wife's mother/father, sister/brother, etc? Or does she only have to fill in her DOB, place of birth, etc on my form?

Wait - there are two forms that need to be filled out. One for YOUR additional family members, and one for HERS.

Does she need to fill out a IMM 5406 or is it another one of the forms?

Hello all:)

I'm new here and also have a question about this form, IMM 5406.

My girlfriend is a Canadian citizen and is sponsoring me, so do I put her down as my common-law partner? What's throwing me is the phrase "whether accompanying you to Canada or not".

I want to confirm with newer common-law applicants that they submitted TWO IMM5406 Forms (ONE for the applicant and his or her family members and ONE for the sponsor and his or her family members)

Clarification would be fantastic!

This form does not need to be completed by the Sponsor. If you look at the checklist for documents to be sent, you will see that this form is not on the Sponsor's checklist.
