Hi,

I am sponsoring my wife, I am a Canadian citizen. We have no kids, my wife also has no kids. My questions regarding IMM 5406 - Additional family information is as follows:

1. The first column says "Name" which has to be written in english and our native language. I am not sure if "name" means full name as it including first and last name or do we just need to write the first names.

2. In section A, under marital status, does my wife need to write "married - physically present" beside her parent's names too or is it just required beside her name and my (her spouse) name?

2. I understand I, as the sponsor, do not need to fill this form our correct? My name will appear in the "spouse" column in my wife's form.

1. Write the full names.
2. 'Married' is enough for the parents.
3. Right. The sponsor does not fill out this form. You will be listed in the spouse column.

@canadianwoman, you think it is okay if I will just write "Married" under my Marital Status? The guide says it should be "Married - Physically present". It fits though under my husband's as the box has more space. But mine does not. the word "Married" is what only fits. I am thinking though of maybe just handwriting the answer after I printed it. Or, will I have to attach a separate sheet just for that? :)

I printed "married - physically present" with a pen after printing. And for the sponsored person's parents, just "married" is fine.

If it doesn't fit, just write it in by hand.
