If passport requested to Canada, would it be refused.

I applied to a tourist visa for my wife from Colombia. its been 3 weeks since we did the application from the visa center there. They said we would be notified in about 3 weeks. we received notification that her passport has been sent to Canada.
My question is what are the chances it would be refused. Wouldn't they have refused it before being sent to Canada?
Also how long does it usually take from there to be sent back?
We did out application not online.

I know we should be patient and wait but was hoping someone could shed some light on this

Can it be refused? Yes, it can.
What are the chances? No one can tell, thus they are low.

Time? in the VPC it takes from 2 to 5 working days, add to this mailing time.
