well, i am very confused now, because I found this http://www.cic.gc.ca/english/resources/publications/dmp-handbook/appendix-02.asp
and it is a form for Outland application, so i guess there is a form????

Fill out the appendix C included in the guideline for outland application, bring it to the doctor, but call the doctor ahead to see what they need from you ( some require 2 pictures, other 4, etc), and you'll be fine!

Indeed, we also filled out this Appendix C form to bring to the doctor.

The CIC website sure doesn't make it easy to let new applicants know this is needed, nor where to download it.

edit: Looks like from the link i previously posted, even Appendix C form is no longer required:
IMM 1017B: Upfront Medical Report: Client Biodata and Summary (not available on our website as panel physicians will have these forms in their office. With the implementation of the new IMM 1017B, Appendix C will no longer be required)
