Upfront medical as well as police certificate are actually a requirement for ALL spousal applications if you look at the checklist, however, there is a wide consensus that you can provide those later in the process when requested by CIC and I don't believe there were cases when applications were returned because of that. Are you aware of any cases that applications were returned?
Hmm, you're right - I guess it is required for all upfront, thought it depended on the VO (just that I read some people doing it after, and thought they had a choice in the matter).

You can explicitly request and receive an X-Ray deferral as a pregnant applicant. See section 4.6.1 of the Panel Physicians' handbook for the process. Essentially, you go through everything but the X-Ray, and then complete the X-Ray after delivery.
So if I got it right from that handbook, I would do everything but the X-ray; but anyway the results would be submitted altogether only after birth (i.e. the date of the medical exam should be the one of my X-ray!?)... correct? But then it should not matter when I do the rest of the examination (upfront or together with the Xray) - unless it is this deferral letter by the panel physician which is important...

Yes, the deferral letter is important. The physician will submit the medical results right away along with the X-ray deferral. That is what tells CIC to put your app ON HOLD while awaiting completion of the medical after the pregnancy.
