1. download the forms from fbi website and print them (preferably on a thick paper)
2. leave that space blank for officer taking prints

I actually filled in the details of my current employment but i don't think that section is relevant to the pr application.

some companies are charging 4500 Rs per person for it! which is ridiculous.

I got it done from the government finger print bureau(Kerala). Fee was Rs.1000/- which had to be paid at the government treasury.
You could locate the one in your city/town and get it done that way to avoid risk.
