How to fill in the form for FBI PCC in US?

Hi,

I am trying to fill in the form for the FBI PCC in the US. While filling the form I have some doubts, like what should be Applicant's home address(is it supposed to be Indian address or my current US address) and also under mail results to address what needs to filled in C/O and ATTN?

So the applicant mail address and mail results to address are same if I want the results to be delivered at the same place
