Hello, everyone!

I was requested by HKVO to provide:Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature Please complete Question No 7, and 8 since 18th birthday. Do not leave any gaps in any period of time. Item No. 12, addresses.

Does someone have similar experience to tell me if I could submit the needed documents via Case Specific Enquiry Portal.

Thank you.

No, not yet. My spouse applied for eTA early on, as it was not clear how long it would take to get DM/COPR, and we were planning to land in the summer and wait things out if the application was not approved yet.
Hi Cheng,
Just got an email after 15 months to request more documents.
1. IMM5669 from age 18 - already done
2. IMM5645 - don't know how to fill in the type Visitor, Worker, Student or other - should be other. Should I put my daughters as Canadain here??
3. Police check - already done
4. Re-do medical exam
5. Passport with Bio-data

You are right to re-do the IMM5669.

Yes, submit via case specific enquiry. If you are concerned, it won't hurt to send in the original via normal mail. But they'll use the one you uploaded.

Congratulations. Your case is moving forward.

It's strange you are asked to fill in IMM 5645 for PR. There's already a IMM 5406 which fills out the same information.

The CIC website does not refer to 5645 as required document for spouse sponsor PR application. Did HKVO make a mistake? To be on the safe side, just fill it in as not to delay your case.

Is your wife applying for visitor extension? That could be the reason.

If not, then if I were in your situation, I would tick "other", and then write an explanation.

Dear Cheng9999, thank you very much for your positive answer.

My concern is just because the description of "bearing an original signature" mentioned in the email. Anyway I have already submitted the requested document through case specific enquiry.

We had to re-do it also, and uploaded it. I then sent the original to HKVO. When I ordered GCMS notes, it indicated that the form was received on the upload date, and case officer proceeded to act on the case. In fact, there was no mention of the original which I sent via regular mail at all.

My thought was that since I had to re-do, and earlier the better, uploading it is the fastest. In our case, we missed this instruction as the email was auto-deleted. By the time I learned we had to re-do 5669, it was more than 3 months.

It makes me feel relaxed a bit. Hope to come to the stage of DM after this submission.
As my medical will be expired by the last day of July, I am afraid that HKVO may ask me to re-do medical.

Yes, my wife is staying in Canada and need to extend her Visa.

Just curious for those that had their interview, how much additional chat log and pictures did you bring to the interview? Since I don't think electronic devices and smart phones are not allowed in the interview.

Anyone via HK VO who sent in August 2015 already received DM?

I brought chats n photos that were taken after the application sent in. However, they didn't ask for it. And u r correct, electronic devices were not allowed.

Hi,

I know that we can bring out a maximum of 5k US$ currency or 20k RMB. Now, my question is that im going back to Canada with my wife and a baby. Does the baby count as a person too???

hi everyone, what can I do while waiting for the appeal record? It is been almost 5 months and my appeal record is still not received. Is it normal?

What exactly will the VAC in Beijing send to my wife? My understanding is it will be just the passport with VISA and the COPR.

I ask because my mother-in-law seems to think we cannot open what they send or my wife cannot go to Canada for 16 months. I have no idea where she gets this idea from. We must open it, or my wife wont be able to get on an airplane even.

She has silly ideas. But happy mother's day to all mothers anyway. :D
