I just got my Passport back with Visa on it from the VO

Congratulations William and Afroditi! Great Christmas present indeed! My husband got laid off for Christmas So we're debating if he is going to stay in Italy with his brother and wife and new baby or return home to Albania to spend time with his mother and father prior to coming to Canada.

2014 is looking great for you both! Congrats again and keep in touch!

Hi everyone, I am currently in Italy with my husband trying to get him to sign all these gosh darn forms!

IMM 5669 - Background Declaration and IMM 5406 Additional Family Information

It says they must also be filled out by the Spouse. But not if the spouse is the sponsor right? It seems repetitive to the Sponsorship Forms.

Did you submit 2 copies of these 2 Forms?

Who needs to fill out the forms IMM 5669 & IMM 5406?

The principal applicant completes these two forms as well as any dependent applying for PR to Canada - Family Class.

The sponsor, as a Canadian Citizen/Permanent Resident, do not need to fill it out because he/she is not applying to immigrate to Canada.

In our case, when we sent the forms, we just sent one copy to Mississauga processing centre.

Awww thank you! He will be returning to Albania and focusing on getting his high school equivalent to assure better employment opportunities for him in Canada.

I am home now, not sure when I will be able to visit next

Did any of your partners get their up-front medicals done through eMedical?

My husband's form has no results on it and says that he must confirm via internet his willingness to share the information with CIC. It's weird.

Anybody else have a similar document?

It is good for your husband to get the High school equivalent, it will give him more opportunities to find a job in Canada.

With regards to your question - Did any of your partners get their up-front medicals done through eMedical?

We simply contacted a medical practitioner approved by CIC and made an appointment. We did the medicals up-front. The medical practitioner can be anywhere in the world.

The document which the medic handed to your husband after his exam contains a barcode and it enables CIC to connect up to the medical results of your husband's file. I am unsure how the internal procedure of CIC works in detail but sure enough the medical results will reach your Visa Office without further action by you guys. It is all done by CIC electronically and all you have to do before you send your application package to Mississauga is to be sure to enclose the form the medic gave to your husband after his medical exam.

Ok that sounds good. Just wanted to make sure he didn't have to go online somewhere to authorize CIC to review the results.

I assume submitting the document will be sufficient authorization.

You can pay the fees online. Preferably all at once to avoid delays later with your application.

Payment for sponsorship............$ 475
Principal applicant..........................75
Right of permanent residence........490
_________
TOTAL CAD.......................................$ 1040


If you decide to do two separate payments:

You should pay at least the payment for sponsorship $475 and the principal applicant $75 (TOTAL $550), and then, do the payment for the right of permanent residence $490.
