I just received my ITA via Express Entry in last round about two weeks. I did the upfront medical exam last Monday, but the staff in the clinic entered the wrong information, specifically my name and country of birth. As a result, I had these personal details wrong in my e-medical information sheet. These details are available on my passport though. The clinic has contacted IRCC to rectify my personal details, but couldn't provide me with the new information sheet with correct information. On my side, to submit the application, I need to upload the upfront medical exam information sheet. The question is, shall I upload the information sheet provided by the clinic but with the wrong personal details?

The same thing happened with us where the country of birth was mentioned incorrectly for my child. However, i submitted an LOE along with the medical information sheet and everything worked fine and my medical was passed. However, mine was just for the country of birth and not for name.
 
If the clinic has already contacted IRCC, do you have any documentation from them that they have done so? Even an email? If they can't get you the corrected form, I would upload a LoE explaining the situation and provide the statement from the clinic that they have contacted them to rectify the information. The reason I ask for documentation of some form is that just writing a letter with no proof doesnt hold as much weight. If you provide proof that what you are saying is genuine then I think it's more likely to be accepted. But, I am also just a normal person, and I have no idea what is in IRCC's head.
