In the last column, they asked "Who performed the ceremony if applicable"
What should I put for our ceremony event?

if you had a formal engagement ceremony.....like been introduced to your spouse or partner family......you could fill your spouse or partner father (or mother) for "Who performed the ceremony".......it depends on what kind of ceremony.........and note the Q....says if applicable.

I'm not sure how you think anyone here can tell you who performed the ceremony. We weren't there.

if you had a formal engagement ceremony.....like been introduced to your spouse or partner family......you could fill your spouse or partner father (or mother).......it depends on what kind of ceremony.........and note the Q....says if applicable.

So it doesn't mean that I should mention the registered officiator who performed our ceremony in legal format?

Put the name of the person who performed the ceremony. Really, this is a pretty basic and clear-cut question

The person who stood in front of you while you said your vows. The officiant. They don't care about a "financial support person." They want to know which person was legally or religiously responsible for performing your wedding.
