Help for Medical Surveillance Form

Hi there,

Can anyone help me about this Medical Surveillance Form? I didn't really know how to do this. Do you mind to enlighten my thoughts about this? What should I do? I'm here in NL and the embassy just emailed me to send them my passport to attach the visa but they will also send me this Medical Surveillance Form. May I know what this really mean?



Below is the email they sent.

In order to enable us to process your temporary work permit, we require:

- your original passport for visa issuance, please be advised that we will send the Medical surveillance form again once we have received your passport, please complete it and present it upon arrival at the port of entry.

PLEASE REFER TO YOUR FILE NO. IN ALL CORRESPONDENCE.

PLEASE PROVIDE THE REQUESTED INFORMATION/ DOCUMENTATION WITHIN 30 DAYS OF THE DATE OF THIS E-MAIL/LETTER. A FAILURE TO PROVIDE THE REQUESTED ITEMS MAY RESULT IN THE REFUSAL OF YOUR APPLICATION.


Does it mean that within that 30 days, I should send the form that they need? Or do I have to go back to NL after I'm done with this? I really need your help. Thank you so much!

Hi,
At present you have to send your passport within 30 days of e-mail date.
After visa stamping they will send back to you with your personal information & passport number on it. The Medical surveillance form, which you have to complete and keep with you, at landing time you have to present this form to the immigration officer with your passport. They will stamp and will give you back with instruction.

Hi, evz,

NO, they will not send you back. You have to report the Medical Surveillance Center in Canada for further tests. It is only monitoring measures to make sure that you don’t have active diseases or it won’t become active in Canada. If all tests are negative you will be clear, if it is positive (hopefully which will be not) you may go under treatment or retest after 6 months. By the way do you have an old Scar in your x-ray? or old …. diseases history?

If you have any private or confidential matter then you can PM me I will reply you in detail. Generally speaking you don’t need to be worried about Survellance or sending back. Please Send your passport and prepare for landing.

i got this tb 2 years ago when i was in the philippines..the doctor give me the medicine to take in 6months treatment..when i go here in NL, the doctor said that i still have this spot and on my latest medical which is one of the important requirement to come over in canada, they still found out that i have this inactive tb..they compare my last chest xray to my latest one.

sorry i did confused you on my last question..i just type it wrong..what i'm trying to ask is that, can i still work even if im still under this kind of surveillance? thank you so much.. i don't know how yet to send PM since i'm still new here.. i'm still trying to figure it out, how. thank again.

Hi, evz,

There is no problem. This spot will be always appear in the x-ray and never disappear or change. After treatment you are perfect and healthy like anyone else and you can work as normal even during surveillance report or investigation.
Surveillance will be the x-ray to compare with latest x-ray film, (as you have done outside Canada). If no changes see you will be clear.
Note: Do not forget to bring all your reports, x-ray films, C.T. Scan and sputum reports with you when you are landing in Canada. It will be easy for physician to get history and can compare the all films and see the spot position.

Help for Medical Surveillance Form

I have recently received my PR with medical surveillance for inactive TB.Presently I cannot stay more than a week in Canada.So Anybody can please advise me regarding how to go about my surveillance?
1)Shall I schedule an appointment this time and report to the public health professional? How long will it take for this?
2)If I leave Canada before medical surveillance is completed,will I be allowed to reenter Canada at a later date; even if this second date is outside my period of expiration of visa?
3)If I leave Canada before medical surveillance is completed,will I still be getting my PR card?

Please help and please reply soon as I am very anxious

(After landing you will receive the contact details of Medical surveillance office call them and follow the instruction, May it will take few days to get the x-ray report and fax the result to them, if clear you will be not needed any other test)
2)If I leave Canada before medical surveillance is completed,will I be allowed to reenter Canada at a later date; even if this second date is outside my period of expiration of visa? ( NO, you may be refused for entry or ask to do the medical again, or may be kept under isolation tell the test result prof you are negative, No one will suggest to do so)
3)If I leave Canada before medical surveillance is completed,will I still be getting my PR card? (yes you will get PR, but it is risky to come back)
