If so, would you please provide some details as to why you/they failed, and if you/they were given a chance to re-take the medical exam... or is it a done deal for good sort of thing?

Search this forum for the words "fairness letter" if you want to read about cases where individuals may have been refused due to failing the medical. CIC sends the fairness letter when they plan to refuse a application due to a medical issue and gives the applicant an opportunity to respond before making a final decision.

Yes, people do. They estimate your yearly medical bills.

The cost threshold is determined by multiplying the per capita cost of Canadian health and social services by the number of years used in the medical assessment for the individual applicant. This cost threshold is updated every year.

Effective December 1, 2014, the updated cost threshold is $6,387 per year. This figure is usually multiplied by five (unless the anticipated length of stay is shorter than five years or there is evidence that significant costs are likely to be incurred beyond that period, in which case the period is no more than 10 consecutive years). This results in the legislated threshold of $31,935.

If you are under those limits, you are fine. If you are over those limits, you can try to argue that you won't be, and have an opportunity to provide a mitigation plan.
