Isn't the instructions laid out on the letter? Post the letter so we can guide you better. It is usually indicated in the letter they gave you whether photos, or other documents are needed. Usually its just the Medical Request or is it medical receipt? Clear up your questions they are quite vague so we can help you better.

Letter says:
Please be advised that your immigration interview is waived pending receipt of all the documents mentioned in this letter and passing medical exams. You may now proceed with the medical exams. ......
You will find medical instructions enclosed. Proceed immidiately with the medical examination and ensure that the DMP you have chosen is in the DMP list. Please submit legible copies of the receipt from the DMP office to us asap as evidence that you have complied with the medical exam requirement. The receipt from the DMP should clearly indicate the DMP's complete name.
Please complete an IMM8 form and return to our office promptly.
