I had an appointment with the doctors for medical yesterday .it all went fine only problem was i recieved only 1 form for my wife sini in the 1st mail from CHC but then they said its ok we have to do all manually and send courier accross to CHC NEW DELHI .so they did our medicals and in 2days time they will send it .but the today morning i got another mail from CHC sending mine and my daughters medical forms .now since i had already done my medicals i called the doctor and asked they said its ok we did it manually and will be sending it like that as the forms for me and my daughter were not their in their 1st mail .
its ok i hope .please send me a covering letter saying so and any other documents to be included in the package or no ?

please send the covering letter today itself so that i can send the passports tommorow

Please do not worry.Everything will be fine.If DMP has not sent medical report of ur wife,please request him/her to send all the reports in a single pack, giving references of the emails recvd by u.And,on ur part,u may also send the copies of MEDICAL REPORT:SECTION A,CLIENT IDENTIFICATION & SUMMARY FORMS + ORIGINAL PAYMENT RCPT, covering medical examination charges rcvd. by the DMP,with all other docs.as asked in ur MR letter.
Cover letter is simple, All that u require is to write details of enclosures/docs. required to be sent.Also, please use barcode stickers to all PPTS+COVER LETTER +ENVELOPE in which the docs are enclosed for sending them to VO.

No even I did not receive the same, I do not think it matters much, a bar code sticker is only to ensure that the Staff at the NDVO do not have to manually enter the file Number details and can just scan the barcode, this is to eliminate any chances of error in manually entering the number.

Yes I received them, the instructions said to paste them on all correspondence as this will help them in speedy recognition of your case.

@prahar,
i dont have any problem with medical reports from DMP only thing is 1st mail which i got frm CHC NEW DELHI had only 1 form of my wife attached ,i took appointment very next day itself and went and did medicals for all 3 of us its that since DMP and i DIDNT GET ALL 3 FORMS they did all the medicals manually and will be sending to new delhi vo .
but then the problem is todays mail inbox i see another 2 forms coming from new delhi vo for me and my daughter ,,i had already finished my medicals i called up my DMP they said its ok they will send the medicals via courier itsel as mine and my daughters forms were not their in the 1 st mail so they did medicals for all 3 of us manually.
just wanted to know if its ok or no?
tommorrow i am planning to send my passports what all should i inlude apart from covering letter ,CHC new delhi didnt ask for anything else as during my initial application itself i paid my RPRF fees etc.

Please do not worry.If the DMP has sent the medical reports of all the family members in one pack,it should not pose any problem. I reiterate that u should send the copies of medical forms of all the family members, issued by DMP + payment recpt covering medical charges paid to DMP.U must send copies both the mails with the docs,explaining the fact that u nd ur daughter had already undergone medical EXAMINATION,as suggested by the DMP,IN ABSENCE OF THE RELEVENT MEDICAL FORMS WHICH WERE RECVD LATER.
I am sure, this should sort out ur problem.
