Hi po, I have questions:

1. kailangan po ba na e-pa certified true copy pa yung mga photocopied documents ko? or is it(certified true copy) only applicable to documents who needs translation? No need as long as nasa English na.

2. Kailangan din ba na mag-draw ng "family tree" (like a genogram) just to help the VO to trace your connections with your relative in canada? No need din. makikita nila sa Birt Certs yung relationship mo sa relative mo.

3. In Generic Application Form question #3 "How many family members, including you, are in this application?" I answered 1, tama ba? kasi wala naman ang asawa at anak.. Yes '1'

I was informed by a friend na nagpre-prepare din ng application sa FSW 2014 na yung agency nya pa daw ang nag-suggest na ipa-notarize o ipa-certified true copy yung mga photocopies ng docs nya. Sa mga nagbayad sa mga agencies para i-assist kayo sa application, please read the instructions carefully sa CIC website regarding which docs should be submitted ORIGINAL or COPIES lang. Sayang lang ang binayad nyo sa agency kung pati sila mis-informed din.

It's best to read this forum and learn from the masters/seniors.

2. How about the ARRANGEMENT of the Generic Application.. kilangan ba na una yung mga barcode yung ma-uuna? like

a. Is it okay if sa "normal" A4 paper ko lang sya e-print or kailangan talaga yung Adhesive A4 paper "sticker"?

b. anu ba yung size sa mailing labels? meron bang specifications yan? Or half of the A4 size lang yung bawat isang mailing labels?

c. lastly, if they are asking 2 self-addressed mailing labels: one in english and one in official language? kailangan bang tagalog ang isa? or both are written in english?

Based on sa other thread, it doesn't really matter if it's sticker or just plain bond paper, they don't use it naman daw anyway lol (which is true, kasi this is our 2nd time applying and last year, we were rejected due to cap filled, they did not use the mailing labels naman haha).

I printed 2 in English.

Question po.. Naisip ko lang.. Kasi nag positive po ako sa koch's infection before (TB). But treated naman na cguro may 2 yrs na.. Minsan yung ibang xray ko upon physical may nakikitang "pilat" or "peklat" basta may mark ng konti dun sa area na affected..but meron nmn times na clear chest xray ang lumalabas. Sabi ng doctor ko minsan daw sa liwanag ng xray un or wat kaya minsan kita minsan hindi. Question ko mag fail kaya ako sa medical exam? May alm po ba kayo n ganito dn case? And may scoliosis din pala ko
