My understanding based on everything I've seen is that there is a brief second eligibility review done at the visa office. Stupid, I know - but I think this is that they double-check to make sure CIO have not made any mistakes.
If you check the current ECAS status list, even for post-June-26 applicants it does say there is a status "Visa office eligibility review complete". But since an application would go straight to "In Process" I think this is only used if an applicant fails the 2nd eligibility review.
CIO's job is to eliminate the largest percent of applicants who are not eligible. I guess visa office's job is to fine-tune what's left.
So, I think if your application says "In process" but you have the "decision made" line on the detail - then IMO you have not failed the visa office eligibility review. More likely that there is either an error, or your case is rejected based on something in the background checks (educational and employment verification etc).
Alas until you hear from visa office all we can do is "best guess" at this stage.
