Guys yung sa RPRF paano ginawa nyo sa check? Nilagyan niyo ng endorsement sa likod such as Name and File # ninyo before submitting to CEM?

Thru text kami nag-register... here is the number 0917 568 4868. They promptly reply naman.. They will give you open schedules and will let you choose which one ninyo gusto paregister... they will then give you a confirmation after you send the information they will be asking from you

PS. I tried to ask for their landline number pero they didnt reply about it.

hi janice. when i registered. may linagay ako na date na gusto namin mag attend ng COA. kasi sa manila pa ang COA. dito pa kami kasi negros and we still have to book plane tickets. kasi may promo. until ngayon di pa rin sila nag confirm. do u think ok na un ang registration namin on the date i stated or do i still have to wait for their tx or email back.. thank u very much janice ....

yung sa akin, i followed the advice given ng mga peeps dito. linagyan ko name, app num, and uid sa likod ng managers cheque, gamit pencil.

then, i photocopied yung medical request and rprf fee, tapos sinama ko sa pag send nung managers cheque.

kanina pa kasi ako nag register thru email.. di nag reply until now.. so i tried tx di pa rin nag reply. im waiting kasi coz i have to book the ticket pa goin to manila by today i tried calling the number. di sila sumasagot awwww...

Hi Kikay, we're taking April 6 for our COA. My husband and I registered thru text, then they sent me a text message for the available schedules, then text back ako with our details and preferred date. when is your preferred sched?

Nasa amin na yung original imminization record. Do we need to photocopy it and papirmahan sa pedia namin, then pass these photocopies to Nationwide?

Tiningnan lang nila yung original and got a photocopy
