sa personal net worth namin, nilagay lang ng agency yung valuation ng business namin basing on the financial statements, then dinagdag lang yung POF na cash, total in peso then may conversion to CAD$, since sa baba merong portion naman dun kung magkano yung exchange rate at that time
yun lang wala ng iba, di na namin pinalagay vehicle since you need supporting docs pa, no credit cards, pero if other properties included sa pof, kelangan isama yun pati supporting docs

I already have supporting documents like Assessor's declaration of the clinic, farm and vehicles's value (Inspection and Appraisal Report) Naguluhan kasi ako sa agency ko, hindi nya ipa declare ang clinic ko, where in-fact it is one of the supporting documents that i personally own a dental clinic.

How did you support your documents basing on the exchange rate used: in CAD? Did you submit a bank current exchange?

Hi JPMA, actually i was able to fill it up way back on my first application (October 2010), i also filled up each column in peso tapos yung total in canadian dollars. How about the last portion Which is distributed as follows (CDN$):
Funds to accompany me to Canada _______ (Syempre yong settlement fund)
Funds to transfer to Canada at a later date (is this necessary?? since i will bring naman everything what we have upon entering Canada)
Funds already in Canada N/A
Funds remaining outside Canada (this is the value of my farm, right?? kasi i can't bring it)

You can put N/A kung dadalhin mo lahat ng funds

Mas maganda kung N/A na lang din kasi sabi mo ay dadalhin mo rin ung lahat ng funds mo at kung ilalagay mo pa ung farm nyo ay additional documents pa ang ibibigay nyo dahil kukunin mo pa ung selling value ng farm nyo. As long as na-meet nyo ung required funds ay ok na un.

Sa POEA website ay may currency converter dun at pwede mong i-print at i-attach mo sa form.Un na rin ang gamitin mong convertion, ganyan din ang ginawa ko at ok naman sa kanila.

wala ng binigay na supporting ang agency ko with regards to exchange rate, nilagyan lang nila ng rate

if iba yung noc mo na hindi related sa clinic, baka yun ang reason ng agency mo, baka kasi iquestion pa ng cio kung bakit may clinic ka na iba naman noc mo

yes Lizz, private practice NOC 3113 dentist. i have a contract of lease from the hospital where i am affiliated, kaya parang contradicting yong advice ng document analyst ko.

I have downloadd all the necessary forms incl that fee paymt form, ang cc kc na ginamit ng agent ko in paying my processing fee was theirs and i have a copy of it.

QUESTION: pwde ko bang reused ung fee payment form nila with their cc as the payee? does it have to be photocopy or orig? agency owner is based pa in canada...

mas mabuti pa nga kung ganun westpoint, at least sigurado since new application sya talaga, merge mo na lang yung mga jd's nila ladyk and don't forget to include all supporting docs ng clinic ninyo like mga registrations para sure na , di kaya ask mo na rin checklist nila ladyk on what docs they submit di ba? para wala ka ng makalimutan

pls help me with the list of documents that u have submitted including supporting documents not mentioned in cic website...start na new career ko bukas, pag prepare ng docs for 3113 applicant, so far eto pa lang additionals that i have in mind: (since 10yrs na in practice)

@ Westpoint> at last our IELTS were done already yesterday. And it's waiting time again, the result will be out on April 1. I would like to ask some assistance regarding the personal net worth & liabilities on how to fill it up perfectly. I will not expect 100% from my agency, mabagal and because they already know that the client will just wait for their time pace. Kasi bayad na sila eh. I will negotiate a refund or transfer on this.

Kaya yan ng hubby mo.. I suggest he should take the April 2. It's a matter of 2 weeks na review, just know the strategy on how to do it. I will give you this website were you can have your practice test at home lang. www.ielts-exam.net and the link which pinoy1028 gave us.

This is how we did it--
Funds to accompany me to Canada (yup, settlement fund; cash or liquid asset),
Funds to transfer to Canada at a later date (not necessary; assets that would still have to be liquidated and transferred in the future- if there is any),
Funds remaining outside Canada (yup, all assets that would not be liquidated)

No need for certification of bank current exchange rate. basta whatever you declare you should have supporting docs. Pwede ka naman declare marami eh, if you have lots of assets. but, it doesn't follow that you will bring them all. you can always declare that some of your assets will be left outside canada.

About receipts ng mga dental lab, parang di na kailangan kasi sa annual itr may record iyon ng income and expenses for the whole year.

PTR Check photocopy of receipts. If he still kept the previous PTR 3-5 years much better
DTI Check ask your BIR Branch if they can authenticate a copy of it. Mine, i got a new one pero the issuance was still 1996
BIR REG & Monthly percentage Check

convention cert of attendance NOT needed upon submission
prc id Diploma from Pre-dent and Proper Authenticated copy at PRC for Board Certificate and Board Rating

cert of employment as contractual dentist of world vision You should support this in the ITR (if he receives compensation)

i'm afraid you can't because the fee payment form must be original as well as the signature of the owner of the cc.
does the copy of your previous payment form already have the card verification code ng card owner? kasi you'll need it also. when we passed our application October last year card number lng yung needed sa payment form..but ngayon dinagdag nla yung card verification value code (the last 3 or 4 digit number) that is found on the back of the cc.
