It's nothing. It's just a simple form with a table with following columns: Name, DoB, passport number and expiry date, and your eye color and height etc.

and at the bottom they've got a box where you've to write your full address.

It doesn't even ask you to date or sign anything!

That's it!

12 months from MR reports recd date. That's the norm. Nothing unusual.

Ensure your passport is valid for at least a year and a half from the Medical date i.e. the expiry date is 12 months from the medical date if your passport has enough validity. Otherwise it is less.
