Tell them now. But decide what you want to do about the MR first. If your wife wants to do the MR and take the x-ray with a shield, I believe they will allow this and it should be safe (but consult the doctor to be sure). If that is what she prefers you can continue with your application as normal and hope to complete your landing process and have the baby in Canada where it will be born a citizen. You have a pretty reasonable chance of having a visa in time to land. But if your wife gives birth before you land, you'll have to get all your baby's documents, add your baby to the application and get a visa for the baby as well, which delays your application but is not otherwise a problem.
Either way, please note:
1. You cannot add the baby to your application after visas/COPRs have been issued. If you have a baby after visas are issued but before landing they are likely to be revoked and you'll have to start over.
2. Even if you have landed and you are a PR, if your baby is born outside Canada it has no status in Canada and you will have to sponsor it. That is possible, but much easier not to have to do that, so if you decide to land before the birth, stay in Canada until after the birth.
3. Don't try to land with a baby that has no visa!
