Do i need to send my old or expired passport bio pages copy (if yes do i need to include pages where Visa for other countries have been provided?)

u can use VFS for sending of PP to VO.

Last charge reported of 1123 was of 20th,Oct... since than no charge observed...

Hello, DD charge takes about 10-15 days more than CC so be patient. It will get charged during January iA. You can check for encashment of DD from the issuing bank.
