Why did you create this mess for yourself? Its better to give priority to FSW application right now and do it as per what you have submitted in the forms to avoid unnecessary bureaucratic delays. I am sure there would be ways to change your name in passport post you have got the visa/pr and have landed in canada.

SSC certificate in lieu of BC is acceptable. As someone recently reported that her file was rejected for not attaching her husband's BC though provided SSC certificate, so send the affidavit as addendum. It won't harm your application.

Can you please share the affidavit format...

Got a good news to share.... I got my medical request & RPRF fee payments email yesterday.... my time line is on left....

Thanks a lot to all of you for all the support and the kind words to reduce my tension......

now as the application has progressed...so as my questions..... PLS HELP....

1) I got two another emails with UCI no of my wife and my daughter...when I checked that online....it doesn't show any status. (Think it might take couple of days to show something there.......!!!????)

2) I have to pay to RPRF fees....(The link which is contained in the email is as below : )

Fro Fee Schedule : http://www.canadainternational.gc.ca/india-inde/visas/fees-frais.aspx?lang=eng

Just confirming my understanding : I have to pay RPRF fees for Me & my Wife (don't need to pay anything for my 1.5 yr old daughter)

and the amount is INR 26700.00 + INR 26700.00 = INR53400.00 via DD in favour of "Receiver General for Canada" Payable at New Delhi

3) What are the documents I need to send along with my medicals to 7/8 Shantipath, Chanakyapuri - New Delhi ?

Send the cover letter for this "unknown" issue. Provide the copy of old passport also. I don't think it would cause any problem.

Medicals are valid for one year. If you already had your medicals (for student visa, for example) during previous 12 months, you can use it again for FSW. I know Ukrainian couple who came to Canada this Spring to study, and right before that they had applied for FSW - they used their medicals again for FSW when they received MR from Ottawa in Fall. But I don't know the exact procedure. All I know is medicals are valid for one year.

Hmm..That passport was taken 10 years back when agents used to fill the application on our behalf and hence that mess in my passport happened. Thanks a lot for the insight.

Continue to wait brother. 70 days is way too early to expect any progress. You may do analysis on the forum members spreadsheet and see that it takes on an average 110-130 days to receive PER. PER will contain your UCI. Anyways even if you get to know your UCI through repeated calls to CIC you will see nothing on ECAS till you receive PER.
