No one can tell you what happened exactly except NDVO. send them case specific enquiry immediately. You can also cal the doctor and confirm the date that he send medicals.

Thanks for your reply. Doctor confirmed us and infact sent the copy of the report to us. I sent email to NDVO but no reply as such. Is there any other way of raising case query?

Please help.

1) In generic application form updated in nov-14 after validating the form and printing the form, Do we need to write N/A with pen in the blank fields after printing form ? because as per Application guide IMM 7000 there should be no field left blank and for those fields that do not apply to you should be marked N/A.

2) How to attach photographs in application?

The blank fields are highlighted automatically with a grey shade before printing, so there will be no need to write on them. Just leave them blank.
Use a Paper clip to attach your photos with the documents in the same order mentioned in the document checklist.

Has anyone under NOC 3012 Sept.11 via DD received PER yet? When can I possibly receive mine?
I called my bank today and they were already hesitant to contact their branch in New York for inquiry re encashment because they said it's not that easy. Last week my DD is still outstanding as per the reply of my bank's main branch in NY.

btw, did anyone notice that CIC is not updating caps for quite some time. Any idea what could have happened?

U r not late as long as it is before 31 dec , ur NOC 0911 is not gonna fill up fast , and u'l be just fine , regarding CIC , they r unpredictable , lately they seem to update caps only when one of the nocs is reaching cap , otherwise they r hibernating

Answer regarding additional sheets Question.

Just make alignment in word document according to the form with header.
In header add details like Given Name, Family Name, Form Title and Number Question

If you want additional documents for you write your details in header OR your spouse details.

From Title, if you are answering for the form SCHEDULE A
BACKGROUND / DECLARATION then the same in form title.

Number of Question will be 2 if you are going to give answers for 2 question.

In content give the appropriate question number as in the form. If you are answering for the 3rd question in form then give 3 in the content of the additional document.

Actually NEITHER !!! My DD was encashed on 20-Nov and I started work on my PCC from India and US(since it takes 3+ months to get PCC from FBI) so that I have the PCC ready by the time I get a request from VO(hopefully in March 2015).

I could contact CIC Call center but they refused to give me the application number
