Dear all,
I am filling IMM0008ENU generic application form and have some doubts:

1. I have only given name (no last name, no surname, no middle name). Should i leave blank 'Given Name' box or put N/A in it ? As i am writing my name in 'Family Name' box only.
2. I am applying first time to CIC. What to fill in 'UCI' box. leave Blank or write N/A ? This box is not accepting neither N/A nor 'not applicable' as there is some word limit.
3. In 'Contact Information' column, in which box (P.O.Box, Apt/Unit, street no.) should i write H.NO. (House Number) (as my address starts with word 'H.No.___').
4. As I am from India, Can i write my Adhaar number in 'National ID' box. If yes, what is its expiry date ? If no, What should be done with this box ?
5. My 2.8 year old daughter is also accompanying me and she just joined a playway school. What should be filled in box provided for 'Current Occupation' and 'Intended Occupation'.

Please reply...

test CC Charged: 02 October (NOC 4011 as per SS) 05 & 08 Oct entries don't seem genuine entry created & updated at around same time.
Latest DD encashed: 16 September (NOC 0113 as per SS)
Latest PER: 02 September (Member named Samoo2014 NOC 1111 but his entry as per SS is quite mixed up)

1. I have only given name (no last name, no surname, no middle name). Should i leave blank 'Given Name' box or put N/A in it ? As i am writing my name in 'Family Name' box only. - Yes, write Given name in Family Name field and NA or Not applicable in Given Name field. You can check the link below for more clarity.
http://www.cic.gc.ca/english/helpcentre/answer.asp?q=014&t=4
2. I am applying first time to CIC. What to fill in 'UCI' box. leave Blank or write N/A ? This box is not accepting neither N/A nor 'not applicable' as there is some word limit. - Leave it blank.
3. In 'Contact Information' column, in which box (P.O.Box, Apt/Unit, street no.) should i write H.NO. (House Number) (as my address starts with word 'H.No.___'). - No need to write H. No. Just write the number of your house. You can mention H. NO. in the self addressed labels.
4. As I am from India, Can i write my Adhaar number in 'National ID' box. If yes, what is its expiry date ? If no, What should be done with this box ? - Aadhaar card is not UID. Infact, India doesn't have any UID. Just answer it as No.
5. My 2.8 year old daughter is also accompanying me and she just joined a playway school. What should be filled in box provided for 'Current Occupation' and 'Intended Occupation'. - I think Not applicable will be fine.

yes, my main issue is that they say it HAS to be turned in since the age of 18, but for all other docs where there is such clause, they also state "or previous 10 years, whichever is more recent". and with the old app, they kept my usa pc, and i heard that now it takes over three months to obtain another one, so i'm thinking should i start looking for new one or wait for LO to ask for it. but then, i'm concern about the time restrains.
Yups, you are right that they require information for last 10 years BUT if they have mentioned, then in my opinion they can ask for. When was your old application dated and what was the date when your last USA PCC was issued? It seems that you have already applied your new application, in my opinion its better to get a new one from FBI which you said will take three months. Its depend upon the mood of Your VO where he/she can ask or he/she may not. Because, you haven't receive any kind of acknowledgement from CIO that they have kept your previous US PCC from old application. Think otherwise, may be it has been lost!!!! Be little pessimist on safer side.

CPA - Case Processing Agent (rank-and-file CIO emloyee)
PSO - Program Support Officer (CIO managerial staff)
IPA - Immigration Program Assistant (rank-and-file VO emloyee)
IPM - Immigration Program Manager (VO chief)
TAT - turn around time
QA - quality assurance

This information was very helpful. +1 to you! I guess that the IPAs at the LVO are quite slow to process NOC 2172. Been waiting for my MR for more than 2 months :'(

This is wrong. VO considers PCC valid for 1 year unlike CIO.
I've not received my PER so far but certain embassies are asking 3 months time for PCC issuance. Should I start the process now? Please advise.
