4) my employer doesnt issur salary statments but they deposit salary in my bank and entry in bank shows "Salary Received". Can it be subsituted for salary slips. I have printed out the 6 months bank credit statment showing salary received from my banking online portal
5) whem it should expect My PER if I submit my application next week

2) You are right... since you dont have more than 5 dependents, you dont need to send additional dependent form... I applied with my wife and son and did not send the additional dependent form and got PER....
If your consultant talks about to be on SAFE SIDE and doesnt know the actual things that means he is ILLITERATE and and doesnt have knowledge about the process
3) AM not sure whether it should not be older than 3 months or six months
4) Salary stubs are additional documents... If you can get bank statements. Important is Job duties description in the reference letter
5) In 110 to 120 days
