Adding more information if it helps to clear someone's anxiety. My NOC 2174 job duties were on a plain paper (not on Company Letterhead), signed by my immediate supervisor. I submitted an explanation along with letter from HR that company policy prohibits issuing such letter, added salary slips, promotion letters, etc. So, CIC is fine with this approach and you should be able to get PER.

Hi Jaxon, good advise you received from some very good ears, also in your apology letter use words like "unintentional", "folly committed in declaration while forgetting what happened in past", also request for an interview
that'll give them a sense of genuinity from you, 80% chances are they wont call u but even if they do then I think you will get a better chance to clarify!! This is just another advise, rest is solely up to you bro.

But, we didnt include the UCI number which was assigned to him that time in this application and sent already...

WHAT IS YOUR MODE OF PAYMENT

It's CC, but we had problems with cc, our bank issued new credit card with new numbers, without our knowledge.
This was at the beginning of 4th of June charge period (according to FSWP 2014 Spreadsheet) cca 23rd of September.
We acted immediately, and send several e-mails with new payment form (original mailed with fedex).
It has been 3 weeks so far, we have no app returned to our address neither app has been charged.
