On the background declaration form , the boxes are too small and font size too big.

On the education section

I cant fill my whole institution name , type of certificate etc .

Can i type please refer to attached sheet in those boxes?

I only saw some chinese and hongkong ones.

last year when i send my application i wrote
family name : xyz given name : abc

CIC retured my application because of cap got filled but they did some modification

Family name : abc xyz : given name :<blank>

so i followed the same.
Please be sure before sending the application.
I have already send so can't do anything.

It could be a valid point. I thought .gov.bd (http://www.banbeis.gov.bd/es_bd.htm) represents Bangladesh government.

As per the instructions given on the top of the form and guide, yes you can attach an additional paper and enter the information in the format requested. You will need to write you name, name of the form and the question you are answering on top of this additional paper.

How and where do I obtain one?

You should:
-get a cover letter and fingerprint card from the FBI. You can download these from the FBI website or write to the address below and have them mailed to you.
-have fingerprints taken. You can have this done by:
-a local police department or

include a Certified cheque or money order for the fees, payable to the U.S. Treasury
mail the letter, fingerprints and fee payment to:

FBI, Criminal Justice Information Services Division

My application is already on the way ...

As I told you, I followed their instructions to the best of my understanding and wrote NA in her family name as it is blank in her passport.

U can keep in that condition (left blank) or u can write zero manually with a pen...:)

Just once in English is sufficient.

Thanks a lot bro. As for the card, I ordered it through amazon (faster).

So it took you 5 weeks to get the PCC letter or just to process it?

U can fill that form once....Just write manually all the required names in ur native language beside of the tiny free space of the each box (with the same english filled names)...:)
 
If I am planning to get WES assessment for my Bachelors only and not my Masters, is there any point mentioning the masters on highest level of occupation in the generic application form IMM0008ENU_2D or on Schedule A Banckground declaration?
