No need to send original copy, if they wish they will ask for original for varification. THEY WILL NEVER RETURN YOUR LETTERS IF YOU ARE GIVEN UCI, SO PLEASE DO NOT SEND ORIGINALS.

Please advise about COVERING LETTER. If writing a covering letter about a missing document do I need to write it on a stamp paper or normal paper? Also should I get it attested from a notary or will my sign and date be enough. Lastly where does one put the covering letter? At the top of the documents or at the place of the missing document? Thanks

Hi, your story looks like my story, I had applied in 2010 and never received mail from CIC for submitting documents. Probably it went into spam and I only received closure letter 4 months later. In my previous application I had written place of my parents birth as Pakistan, however I realised this time that in 1943 when my parents were born there was no country named Pakistan. So this time I have written India. So my suggestion would be you go with what you feel is right now. If you had made a mistake in past, do not pull it along with you but correct it.

It is called a covering letter because it needs to be on top (no offense meant)....No need for notarisation, just plain paper with all your application/personal details.

I have two questions,

1) PCC, I have spent 26 years of my life in one state in India and since last 7 years I am in another state. Now Passport office can issue PCC, Do I need to go for two PCC? one from Old state and another one from current one ?
2) I have my birth certificate in local language and I did translation with affidavit. I sent notarised English version with Affidavit. Is it enough or I missed to send photo copy of local language of birth certificate.

I hate to say that but a true copy of the original BC is indispensable for processing the application.

1) I think you should give affidavit if lost documents are very important like BS, offer letter. For other documents covering letter on normal page is fine.
2) Covering letter should be before the documents where you are justifying your case. Eg. In case of JOB you should put it before your job doc start.

1. Your PCC must include all address changes (changes in police jurisdictions), however, you dont need to apply for 2 PCC. Also, it helps, if the passport has one of these addresses, the verification will be done for the other address. My wife received the PCC immediately because the address was the same on the passport.
2. The copy of the local language BC must accompany the translated, notarised version.
