The fact is that fee can still be charged without CVV number. It just needs CC number and expiry date. Not sure which form you are referring to. When I reached the branch, they provided me a regular DD request form (similar to what you get for Indian DD). They also had a declaration form, which I filled up by hand.
So, I wrote "Immigration" under the Purpose header. There wasn't any purpose code. Are you trying to fill it online somewhere? If that's the case, i'll suggest chuck all that and just go to a branch (NRI branch or Overseas branch) and get it done in person.
