Hi,

How do you guys fill the additional family information form (IMM5406) for spouse.

There are four names required in section A

1. Applicant Name------
2. Spouse Name------
3. Mother's name------
4. Father's Name-----

when spouse is filling up the form who's name will be on the applicant's name?

When you are filling you are the applicant, your spouse is your spouse . When your spouse is filling, s/he is the applicant and you become her spouse. Mother/Father's name is the respective applicant's.

Does it require to sent photocopy documents attested by Notary public? In the checklist it is mentioned "copies". I do not find any reference where it is mentioned that document should by verified by Notary public. Please share your experience.

I notarised all my photocopies, every single one. That way, they are copies but are as good as originals. Got this tip from a friend who did same and is now in Canada as a PR.

NO. You can write in Bangla with pen after printing the form. Or can add additional page.

Actually, that's not accurate. On the form, near the top, there are two check boxes. One check box for the primary applicant to tick before filling the form. The other check box for the spouse is to be ticked on another copy of the form before the spouse fills it.

The primary applicant remains the primary applicant on both forms, the spouse remains the spouse on both forms as well. The forms are differentiated by which check box you tick.

If i have multiple FD's in my name... Please suggest i just need to provide the photo-copies of FD certificates issued by the bank as a proof of funds or a balance certificate for all the fd's from bank is also mandatory?

It will be a good idea if you pls check ADDITIONAL FAMILY INFORMATION form again and find the two check box.
