In the medical request form, nowhere the passport number will be mentioned. However, at the time of ME, they ask for proof then you can provide your new passport. That will not be any problem. Just get the medical panel to notice that this is your new passport (carry both old and new).
Passport is just for correct identity to verify if they are doing ME for right candidate.
While dispatching the documents, you can mention that in a cover letter. Hope this helps;
