I wasnt quite sure if I had to register first or not.. I did eventually. then proceeded to pay.. but the receipt only captured my name. No date of birth, no Client ID number, no address. Is it ok to send it like that or am I allowed to fill in the blanks in ink?

Hi, I didn't fill anything. I just printed it out and sent to them. You can leave one blank, fill one out and send both for your peace of mind, cheers.

Exactly what I did in my time. I filled one and left one clean (fresh off the printer). As a matter of fact, there is no information you are going to fill in that AVO does not already have in your case file. So just be cool.
