Thanks tbaba! Just called them; very pleasant-sounding lady on the phone informed me that earliest available appointment date is next week, and prices are N28k/adult and N20k/child (our kids are below 5). Additional requirements per person are:
* 2 passport photographs against white background (no specific size requirement)
* international passport (original + photocopy of data page)
* any relevant specialist reports (original + photocopy)
* any ongoing medication (the actual drugs, not doctor's prescription)
* any glasses and / or contact lens
* ATM card for POS payment
* CIC medical forms
You can eat before coming.
Note for the ladies: you should not be on your period on appointment day.
On the day of appointment, arrive between 8am & 10am; arrival after 10am means you will not be attended to that day.
