On question (12a) should I write 1212 - Supervisors, finance and insurance office workers

And on (12b) 1212-Bank Clerks Supervisors

Or I should use 1212-Bank Clerks Supervisors on question 12a and 12b?

Don't let the Job tittle confused you...
The most important thing is to get the the Job descriptions right for your interested NOC with reference to your Letter of reference and the descriptions listed under 12b.
